FOOTBALL TRAVEL RELEASE FORM

Students that are driving themselves to their athletic events, may not drive other students to or from the event.

Please submit Required Athletic Documents to ensure athletes are cleared in time to participate in their sport.  Paperwork is required for each season/sport.  If you are interested in participating in more than one season, paperwork must be submitted by the due date for that season.
