SPORTS MEDICAL CLEARANCE PROCESS

SPORTS PHYSICALS

Sports physicals are valid for 365 Days. If your Preparticipation Physical Evaluation Medical Eligibility Form that we have on file is more than one year from the first day of practice/tryouts of your sport, a new one must be submitted.
**New Procedure** Click HERE for the updated physical forms! Only the “Preparticipation Physical Evaluation Medical Eligibility” form needs to be submitted to the school nurse(not the entire physical). The Physical Exam form and the History form are for your child's physician to keep on file.
HEALTH HISTORY UPDATES

Must be submitted for each season of participation via the PARENT PORTAL IN GENESIS if your physical on file is dated more than 90 days from the start of the season.
All Paperwork must be submitted in person and cannot be accepted via mail, fax, or email
Online Sports Registration
For All Students Grades 9-12:
NEW: Specify the one sport you are interested in. This can be changed at a later date.
PARENTS:
Step 1: Log into your Parent Access account (The “Parent Access” icon is on our website). whrhs.org
Step 2: Select the “Forms” tab.
Step 3: Then select each of the following:

2024-2025 Releases (Steroid, Concussion, Sudden Cardiac, MRSA, Sports Related Eye Injury, Opioid, Indemnification Clause)

2024-2025 Guardian Code of Conduct
Step 4: Select the answer button to acknowledge each of these forms and your agreement. You must repeat this process and acknowledge each of the above forms for registration. Select “Update Answers”. You must be sure to select the 2024-2025 school year in each drop down box to confirm your information is updated..
Student Athlete:
Step 1: Students must log into their Student Access account and select the following forms and acknowledge their agreement to each by checking the answer button.

Step 2: Select the checkbox that indicates the form is ready for final submission and make sure all information is correct.
Step 3: Select School Year 2024-2025
Step 4: Read all Consents & select “Update Answers”.

Returning Student Athletes:

Please submit a Health History Update. This form must be submitted for each season of participation via the PARENT PORTAL IN GENESIS if your physical on file is dated more than 90 days from the start of the season.

This is required a minimum of two weeks prior to the first day of practice. If you do not submit this paperwork, you will not be eligible to try out, practice or participate. You must submit a Health History Update for each season you wish to participate, by that season's paperwork due date. (i.e. one by the due date for fall sports, one by the due date for winter sports, and one by the due date for spring sports. The school nurse needs to review and enter each Health History Update that is submitted. Please allow time for the nurse to enter this information.
