Paperwork and online consents are due a minimum of two weeks prior to the first day of practice.  Winter sports physicals are due:  Ice Hockey,- October 21,    Bowling and Swimming- October 28.                   Basketball, Fencing, Wrestling and Winter Track: November 11.

How to Sign Consent Agreements  

If you do not hand this in on time, you will not be eligible to tryout or participate.

Practice Start Dates: (Paperwork must be submitted at least two weeks prior to these dates)

FALL 2019:

  • Cheerleading-June 4, 2019
  • Football-August 12, 2019
  • Cross Country, Field Hockey,Gymnastics, B/G Soccer, Girls Tennis, Girls Volleyball, Ultimate Frisbee

WINTER 2019-20:

  • Ice Hockey-November 4, 2019
  • B/G Bowling, B/G Swimming-November 11, 2019
  • B/G Basketball, B/G Fencing, Wrestling, B/G Indoor Track: Try Outs  11/25-11/27, Practice starts: 12/2.

SPRING 2020:

  • Baseball, Softball, B/G Lacrosse, B/G Spring Track, B/G Golf, Boys Tennis, Boys Volleyball, Ultimate Frisbee will all begin on March 6, 2020.

 

Online Sports Registration for Parent and Athlete

For all students grades 9-12

Parent: Please follow the steps below to register your child for a sport.

Step 1: Log into your Parent Access account (The “Parent Access” icon is on our website). whrhs.org

Step 2: Select the “Forms” tab.

Step 3: Then select each of the following:

  • 2019-20 Guardian Releases (Steroid, Concussion, Sudden Cardiac, MRSA, Sports Related Eye Injury, Opioid)
  • 2019-20 Guardian Consent to Athletics
  • 2019-20 Guardian Code of Conduct

Step 4: Select the answer button to acknowledge each of these forms and your agreement. You must repeat this process and acknowledge each of the above forms for registration. Select “Update Answers”.  You must be sure to select the 2019-2020 school year in each drop down box to confirm your information is updated.

Student Athlete:

Step 1: Students must log on to their Student Access account and select the following forms and acknowledge their agreement to each by checking the answer button.

  • Athletic Releases
  • Athlete Code of Conduct

Step 2: Select the checkbox that indicates the form is ready for final submission and make sure all information is correct.

Step 3: Select School Year 2019-20

Step 4: Read all Consents & select “Update Answers”. You must be sure to select the 2019-2020 school year in each drop down box to confirm your information is updated.

PAPER DOCUMENTS THAT ARE HAND DELIVERED TO THE HEALTH OFFICE:

Incoming 9th Grade and New Students

Paperwork must be hand delivered by the athlete, or preferably a parent.  No emails, faxes or interoffice submissions will be accepted.

Mandatory Concussion Impact Test for Freshman, Juniors and New Registrants participating in Athletics. Please click here for at home testing instructions.

9th Graders and new students require a complete Sports Physical Packet.

If your child has a medical issue, you must complete a health care plan by accessing https://www.whrhs.org/whrhs/health-office/forms.

Clearance is not complete until the school physician approves the submitted physical packet.

If your child wishes to participate in additional sports in following seasons, you must submit a Health History Update for each season.

Returning Students

If you have had a current sports physical within 365 days of the 1st day of practice, please submit a Health History Update a minimum of two weeks prior to the first day of practice. If you do not submit this paperwork, you will not be eligible to try out, practice or participate.   You must submit one Health History Update for each season you wish to participate, by that season's paperwork due date. (i.e. one by the due date for fall sports, one by the due date for winter sports, and one by the due date for spring sports. 

If you have not had a physical within 365 days of the first day of practice, you will need to submit a complete Sports Physical Packet a minimum of two weeks prior to the first day of practice.  If you do not submit this paperwork, you will not be eligible to try out, practice or participate.

SOME HELPFUL TIPS TO HELP ENSURE YOUR PAPERWORK IS COMPLETE:

  • Vital signs must be completed and within normal limits.
  • Visual Acuity must be completed. Vision must be screened with eyeglasses if prescribed.
  • The appropriate "Cleared" box must be check by the physician.
  • "Date of Exam" is the date that the actual physical was performed, the the date the paperwork is written.
  • Physician's stamp is required on each page near the Physician's signature.
  • All Health Care Providers are required to complete the Professional Development Cardiac Assessment before performing any student-athlete's physical examination.  COMPLETED CARDIAC ASSESSMENT PROFESSIONAL DEVELOPMENT MODULE must reflect the date that the physician actually took this professional development course, not the date of the physical exam.  A copy of the professional development certificate should be attached if available.
  • All "yes" and "no" items must be answered on the Health History Form. If a response is "yes", an explanation must be given that indicates the issue has been resolved, including the date the issue was resolved.  MD clearance notes may be needed and should be included.
  • Signatures are required of BOTH the parent and the athlete.
  • The date of the physical must be within 365 days of the first day of practice/tryouts.
