Fall 2026 Sports registration and  paperwork submission will open on June 1, 2026. Please bring all sports exams to the school nurse or the athletics office in Room 126.  If you have a care plan, that must be included with your exam as well.

Students and their parent/guardians must complete the athletic consents in their respective Genesis portal as part of the clearance process.

The deadline is July 27, 2026 in order to be cleared for the start of the season.

Beginning 8/10/26-Football, Cheer, Girls Tennis

Beginning 8/17/26-Cross Country, Field Hockey, Gymnastics, Boys Soccer, Girls Soccer, Girls Volleyball

Your approval for clearance will be updated in Genesis once the school doctor has reviewed your physical.

GENERAL INFORMATION

 COMPLETE SPORTS PHYSICAL PACKET

Must be completed if your sports physical on file is more than one year of the first day of practice/tryouts.  Only the Preparticipation Physical Evaluation Medical Eligibility Form needs to be submitted to the school. The Physical Exam Form and the History Form are for your child's physician to keep on file.

Please bring your Preparticipation Physical Evaluation Medical Eligibility Form to the main office or the athletic office (Room 126) by July 27th to ensure you are cleared in time for the beginning of the season.

 

HEALTH HISTORY UPDATE-must be submitted for each season of participation via PARENT CONNECT if your child's complete physical is dated more than 90 days from the start of the season. (Hard copies will no longer be accepted).   You will also need to submit a Preparticipation Physical Evaluation Medical Eligibility Form if your physical was completed more than one year of tryouts/first day of practice of your sport.

If your child has a medical issue, please have your physician complete a health care plan by accessing https://www.whrhs.org/whrhs/health-office/forms, and ensure a nurse receives prior to the start of the athletic season.

If you need to speak with a nurse regarding sports athletic paperwork, please call 908-647-4800 x 4878. A nurse will be onsite on the following dates/times during the summer:

June 17th 8am-2pm, July 27th 8am-2pm, August 10th 8am-2pm, August 17th 8am -2pm.
