Online Websites/Links to Assist with Preparing for Post-Secondary Opportunities

Division of Developmental Disabilities:

  • Provides public funding for services and supports that assist New Jersey adults with intellectual and developmental disabilities age 21 and older to live as independently as possible
  • Links to regional/local office contacts, SSI and Medicaid eligibility, support coordination, and other services can be found here

Division of Vocational Rehabilitation Services:

  • The mission of DVRS is to enable eligible individuals with disabilities to achieve an employment outcome consistent with their strengths, priorities, needs, abilities, and capabilities

Commission for the Blind and Visually Impaired:

  • This agency’s mission is to promote and provide services in the areas of education,
    employment, independent living and eye health through informed choice and partnership with persons who are blind or vision impaired, their families and the community

Social Security Administration – SSI:

  • This link provides information on how to apply for supplemental security income (SSI) which is a requirement for individuals who wish to receive DDD-funded services
  • Links to completing and tracking an application, as well as requirements, can be found here

Medicaid application/eligibility:

  • This website/link provides information on applying and receiving Medicaid eligibility/coverage, which is another requirement for individuals who wish to receive DDD-funded services

Support Coordination Agencies:

  • This is a link to a continually updated PDF list of DDD-approved support coordination agencies.
  • Individuals and families who are seeking to receive DDD-funded services may choose a support coordination agency, who will assist them in developing an Individualized Service Plan (ISP), accessing desired programs/supports, and managing fiscal responsibilities in regards to maintaining services and supports
  • ill be updated periodically, so it is wise to check it often

Support Groups:

  • This website provides families and caregivers with a list of various support groups they may be interested
  • The list shown may not contain every support group, but it is a good resource to start with. Continued networking with these groups will aid families in preparing for their child’s future after graduation, as well as provide them with needed and/or useful support
